Resolve Program
Resolve is an innovative program which is partly funded by private and institutional investors via a Social Benefit Bond.
We partner with NSW Health and Social Ventures Australia on the Resolve Program to reduce the likelihood of readmission for people with a mental health issue who have been hospitalised for an extended period. The program includes residential periodic care with 24/7 support and outreach for in-home care; in combination with specialised case management and an after hours phone line.
Primary Care Psychiatric Liaison Service
The Primary Care Psychiatric Liaison Service (PC-PLS) model is an initiative by Western Sydney Primary Health Network (WSPHN) and Flourish Australia designed to support general practices to provide comprehensive care to people in a primary care setting.

We value lived experience and more than 50% of the people we employ have had a lived experience of a mental health issue. We believe peer support is a powerful part of the mental health recovery journey.

Back on Track Health (BOTH) Program
This program was established in 2010 to focus on health promotion and prevention, embedding physical health and wellbeing conversations in everything we do. It aims to support people to:
- Maintain a reflective and responsive approach towards their physical health and wellbeing;
- Learn new skills to review, identify and self-manage their own physical health and wellbeing; and
- Take action by regularly accessing primary health care services, particularly GPs, to address their health concerns.
